Personal Information Collection

Clinique Dentaire des Écores Inc is required to collect personal information necessary for the provision of dental care through the confidential medical-dental questionnaire attached herewith for your signature. The confidential medical-dental questionnaire is kept in your dental record, and your dental record is stored in a dental cabinet that is not accessible to the public or is locked.

Only healthcare personnel will have access to your health information; administrative staff will only have access to information necessary for the performance of their duties (billing, appointment scheduling, transmission to insurance).

Retention Period for Personal Information

Your dental record is retained for five years following the last entry or insertion in accordance with the law, and after this period, it will be destroyed in a manner that preserves its confidentiality.
